I use the Sport Satisfaction scale. I wish I knew who the creators were because I want to credit it to them with the information. Given the problem with attachments, I have put it right into the email and also as an attachment. I have found the survey to be beneficial because it gives a picture of a problem that may be foremost to deal with early in the Mental Skills Coaching.
